PI is the main indicator of shunt patency

The pulsatility index is a key metric that the surgeon monitors immediately after the formation of the coronary anastomosis.

Why is this critical? The PI reflects resistance to blood flow in the shunt: a high PI is an early sign of an anastomotic problem on the operating table when it can be corrected. Intraoperative assessment of flow is introduced in ESC/EACTS 2018 Guidelines (Class IIa, Level B) and supported by AHA expert statements; in developed countries, this is the standard for shunt verification, which allows detection 2–4% of shunts, which require revision.

PI = (Qmax − Qmin) / Qaverage over 5 cardiac cycles — estimation of blood flow resistance. Interpret always in a complex: mean shunt flow (MGF) ≥ 20 ml/min and diastolic filling (DF) for left shunts usually > 50%. PI is affected by the quality of the anastomosis, distal stenosis, and competitive flow — the thresholds given are approximate, the decision is made taking into account the clinical picture.
